The Six Core Elements 3.0 define the basic components of a structured HCT process, which in this package for internal medicine, family medicine, and med-peds clinicians, include establishing a transition and care policy/guide, tracking and monitoring progress, orienting young adults into the adult practice, integrating them into the adult practice, addressing key issues in the initial visits, and continuing with ongoing care. This HCT approach, recommended in the 2018 AAP/AAFP/ACP Clinical Report, can be customized for clinics/practices/health systems serving young adults.

To implement the Six Core Elements, Got Transition recommends a quality improvement approach to incrementally incorporate the core elements as a standard part of care for young adults.
