Health Care Transition – Improve It

Got Transition, the National Health Care Transition Center has identified Six Core Elements of Health Care Transition. The six elements mirror the algorithm for best practices contained in "Supporting the Health Care Transition from Adolescence to Adulthood in the Medical Home” published by the American Academy of Pediatrics, American Academy of Family Physicians, and American College of Physicians.

Health Care Transition – Measure It

The Health Care Transition (HCT) Index (pediatric and adult versions) provides a six-indicator, self-assessment to quantify and guide health care transition improvement. The HCT Index was modeled after the Center for Medical Home Improvement’s widely used Medical Home Index.

Health Care Transition - Implement It

Practices may download either individual tools from the links within the Six Core Elements of Health Care Transition or the entire package of tools.  Additionally, any tool or the set of tools can be requested as a Microsoft Office document to allow adaptation and customization to an individual practice, clinic, or project.   To obtain MS Office versions of any of the tools, please complete this order form.
